I have just one small comment on this well written document.  Even though the Guidelines section immediately follows the Issues section, I think it would be good to state explicitly "Don't do anything mentioned as an Issue in the previous section" as a Guideline.  For example, "Commands are issued, rather than hints given" is an Issue, while I don't think any of the Guidelines warn against creating an extension that requires a command-action architecture.
